Endothelial hyperpermeability in vascular leakage

Starling's law regarding the fluid dynamics of vascular permeability was described more than 100 years ago. It is a beautiful example of bringing order in experimental observations by the design of an equation. Although very helpful in predicting the entry of drugs into tissues in terms of diffusion and hydraulic conductivity, it does not sufficiently help in understanding why vascular leakage occurs and how it can be prevented. Still, vascular leakage is a threatening complication in critically ill patients with often-fatal course (Groeneveld, 2003). A further understanding of the mechanisms of vascular leakage is required to develop an adequate counteracting treatment.
