The significance of the E.E.G. for localization and prognosis of supratentorial ischaemic disease: A clinical-pathological study. Preliminary report

This study includes the E.E.G. results of 311 patients with a supratentorial ischaemic cerebral lesion verified at autopsy. Primarily the localizing and prognostic value of depression of faster background activities and polymorphic delta activity were investigated. In subacute Sylvian ischaemia a local depression, parieto-temporo-occipital and centro-parietal delta activity have localizing consequence and include a bad prognosis. On the other hand polymorphic delta activity over the anterior and midtemporal area did not add much information concerning the centre of the lesion ; however the average prognosis was better, especially in cases with preserved background activities. A frontal delta focus (F 3/F4) was only encountered in anterior cerebral artery occlusion. There was a contrast between the entirely unilateral E.E.G. disturbances in subacute lenticulostriate ischaemia and the bilateral findings in thalamic ischaemia. A similar discrepancy found between young and old patients with acute Sylvian ischaemia may cohere with the difference of shifting mechanisms in cerebral œdema at different ages.
